Переход к персонализированной аппаратной мпМР/УЗ фьюжн-биопсии предстательной железы: стратифицированный ретроспективный анализ 322 пациентов с построением модели CHAID
Введение. МР-прицельная биопсия является стандартным методом диагностики рака предстательной железы, однако для снижения риска ложноотрицательных результатов ее дополняют периочаговой или систематической биопсией. Цель исследования. Определить факторы, при которых выполнение МР-прицельной биопсии не приводит к снижению выявляемости клинически значимого рака предстательной железы (кзРПЖ).
Материалы и методы. Проведен ретроспективный анализ пациентов, которым выполнена трансперинеальная аппаратная мпМР/УЗ фьюжн и сатурационная биопсия. Критерии включения: уровень ПСА ≥2 нг/мл и/или обнаружение подозрительного очага при проведении пальцевого ректального (ПРИ) и/или трансректального ультразвукового исследования (ТРУЗИ) и наличие очага PI-RADS v.2.1 ≥3 баллов. В стратифицированный анализ включены факторы: характер биопсии, балл PI-RADS, подозрительный очаг при ТРУЗИ или ПРИ, ПСА, плотность ПСА и размер очага. Для формулировки клинических правил построен CHAID-алгоритм с переменными: выявление кзРПЖ при аппаратной мпМР/УЗ фьюжн-биопсии, балл PI-RADS, плотность ПСА, размер очага, возраст и характер биопсии. КзРПЖ считали ISUP ≥2.
Результаты. В исследование включены 322 пациента, медиана возраста которых составила 63 года, уровня ПСА – 6,7 нг/мл, объема простаты – 48 см³. Первично биопсия выполнена у 241 (74,8%) пациента, повторно – у 81 (25,2%). Выявляемость кзРПЖ при сравнении аппаратной мпМР/УЗ фьюжн и комбинированной биопсии статистически достоверно не различалась (23,6 и 30,4% соответственно; p=0,051). Добавочная диагностическая ценность сатурационной биопсии в отношении кзРПЖ составила 6,8% (0 –28,6%). При стратифицированном анализе установлено, что добавочная диагностическая ценность сатурационной биопсии не определялась при первичной биопсии у пациентов с очагами PI-RADS 5, при PI-RADS 3–4 в сочетании с неблагоприятными факторами, а также при повторной биопсии у пациентов с очагами PI-RADS 5 и неблагоприятными факторами. Разработанный CHAID-алгоритм характеризовался точностью 80,2%, чувствительностью 100% и специфичностью 60,7%. Согласно модели, выполнение сатурационной биопсии нецелесообразно у пациентов с PI-RADS 5 в сочетании с неблагоприятными факторами, а также при PI-RADS ≤4 в сочетании с плотностью ПСА ≤0,20 нг/мл/см³ и размером очага ≤10–12 мм. Использование CHAID-алгоритма позволяет сократить количество сатурационных биопсий на 60,5% при риске пропуска кзРПЖ ≤3,3%.
Заключение. Представленные результаты свидетельствуют о возможности перехода от комбинированной биопсии к МР-прицельным методам у тщательно отобранных пациентов без существенного риска пропуска кзРПЖ, что создает основу для внедрения персонализированных стратегий биопсии простаты.
