Снижение мужской репродуктивной функции: способы и алгоритмы этиопатогенетического и эмпирического лечения основных клинических форм

Уменьшение числа и/или снижение качества сперматозоидов является причиной бесплодного брака в половине случаев. Патогенез таких нарушений полиэтиологичен, часто неизвестен, данные по эффективности лечения по-прежнему противоречивы. На основе анализа последних публикаций высокой доказательной силы (рандомизировнные исследования и метаанализы) дана оценка реальной клинической эффективности различных методов оперативного и консервативного лечения по поводу основных клинических форм снижения мужской репродуктивной функции: гипо- и гипергонадотропного гипогонадизма, секреторной и обструктивной азооспермии, ретроградной эякуляции и анэякуляции, патозооспермии, ассоциированной с варикоцеле, и идиопатических форм. Предложен алгоритм ведения мужчин с данными нарушениями, который может быть удобен в практической деятельности врача уролога-андролога, независимо от уровня оснащения профильного ЛПУ.
